Cancer: Treatment Trends and Help for Survivors | Maryland ...

In this issue, five Maryland physicians describe advances in treating rectal, peritoneal and skin cancers and the role of cancer survivorship programs in caring for those who survive cancer.

Trends in Treating Rectal Cancer

Over the decades, physicians have learned much about how to best treat rectal cancer. Hopkins colorectal surgeon, Jonathan Efron, M.D., and his team are investigating the efficacy of laparoscopic and robotic procedures, endorectal brachytherapy and other state-of-the-art treatments for this disease.

?In the past, we gave radiation therapy after surgery,? recalls Dr. Efron. ?Then, in the 1990s, large randomized European trials showed a better response when patients were given adjuvant therapies before surgery and American surgeons subsequently found it decreased recurrence rates. Today, most patients with rectal cancer get neo-adjuvent therapy prior to surgery.?

Multidisciplinary Approach

Dr. Efron explains, ?We?re using a multi-disciplinary approach. Our rectal cancer center is composed of radiation oncologists, medical oncologists, surgical oncologists, colorectal surgeons, nuclear medicine specialists, pathologists and others. Typically, patients come to us after having had a colonoscopy or sigmoidoscopy that identifies rectal cancer.

?We evaluate rectal cancer using a battery of radiology tests, usually MRI of the pelvis or CT of the abdomen and pelvis,? Dr. Efron continues. ?On the same or next day, the patient can see the surgeon, radiation oncologist and medical oncologist. ?We can coordinate with the patient?s medical and radiation oncologist back home or provide those therapies here. The vast majority of patients proceed with surgical intervention about eight weeks later. About 20% of patients ? those for whom radiation and chemotherapy have apparently completely eradicated their cancer and those who are too sick to undergo surgery ? will receive watchful waiting instead.?

Laparoscopic and Robotic Approaches

?With colon cancer, we know that laparoscopic and robotic surgery results are comparable to open excisions, but we?re not yet sure that that is true for rectal surgery,? Dr. Efron notes.? ?Hopkins is involved in a large, randomized trial across the country, but we won?t know the results for another three to four years.?

Total Mesorectal Excision (TME) Surgery

Studies have shown that total excision of the mesorectum, fatty tissue directly adjacent to the rectum that contains blood vessels?and lymph nodes, provides superior outcomes and has more than halved the recurrence rate. TME is appropriate for patients with tumors in the middle or lower two thirds of the rectum.

Dr. Efron comments, ?In the older technique, the lateral aspects of the rectum were divided and left in. Now, we excise everything. The five to 10 year recurrence rate has decreased to 5-10%.

?In the 1980s, a tumor anywhere in this area would have required a colostomy,? he continues. ?Now, using pre-op radiation therapy, many people are candidates for the preservation procedure. We can often shrink large tumors to alter our operational plan and keep the anal muscles intact. Patients typically get a temporary colostomy but can avoid a permanent one. Less than 1% of patients experience some urinary continence and sexual dysfunction issues as a result of nerve damage.?

High-Dose-Rate Endorectal Brachytherapy (HDRBT)

Hopkins is conducting a pilot study of HDRBT for patients with clinical stage T2N1 or T3N0-1 resectable rectal cancer. This approach, first pioneered in Montreal, uses an anal probe to deliver brachytherapy over a four-day period in lieu of five weeks of external beam radiation therapy. The trial started in early 2012 and results won?t be known for several years. ?We?ve had excellent results with 10 patients to date, and we plan to test it on about 20 more,? concludes Dr. Efron.

Cytoreductive Surgery and HIPEC

For years, the only treatment for patients with peritoneal dissemination of cancer was chemotherapy, which has poor results. Then, in the 1990s, a few surgeons began performing cytoreductive surgery and intraoperative hyperthermic intraperitoneal chemotherapy (HIPEC) at the completion of surgery to destroy remaining cancer cells in the abdomen.?This is a lengthy procedure taking seven to 12 hours that requires a highly experienced surgical team. Cytoreductive surgery refers to the complete removal of all visible cancer or only leaving cancer nodules less than 2.5 mm in size. This is followed in the same setting by HIPEC to destroy remaining cancer cells in the abdomen.

Colorectal cancer with peritoneal dissemination is the most common carcinoma treated by cytoreduction with HIPEC. Others include appendix, ovarian, gastric, primary peritoneal, sarcoma and mesothelioma.

Refer Early, Before Chemo

Unfortunately, patients are often referred late, after they?ve received chemotherapy and multiple surgeries. Armando Sardi, M.D., director of the Institute for Cancer Care at Mercy, states, ?As soon as patients are diagnosed by CT or surgery with peritoneal spread, they should be referred.? Chemotherapy makes the patient debilitated.?

Dr. Sardi says, ?The surgeon has to be prepared to perform an extensive surgery, which may require the removal of several organs at one time. Most of the patients I perform this surgery on have extensive disease and this is their last hope. However, there?s so much data in the literature now that supports cytoreductive surgery with HIPEC, and thanks to the Internet, patients are getting to us earlier.?

Media coverage is misleading, according to Dr. Sardi. ?A New York Times article* described it as a horrendous surgery but it?s not.? ?Insurance is another barrier.? ?Medicare doesn?t want to recognize this procedure,? he adds.

The Procedure

?In the procedure, surgeons remove all of the visible tumors and attempt to preserve as many organs as possible,? Dr. Sardi continues. ?Some 85% of the time, we remove all visible tumors of 2.5mm or greater. Then, the peritoneal area is bathed in a heated chemotherapy solution, which is the most effective way to kill remaining cancer cells. Patients with colon, rectal and ovarian cancer often need follow-up chemotherapy, which we coordinate closely with medical oncologists.? However, there is no data that all patients with cancer of the appendix or mesothelioma benefit from follow-up chemotherapy.?

Following the procedure, patients spend about 12 days in the hospital, start eating after about one week and take two months to fully recuperate.? Most patients are 40 to 50 years old, but some are in their 20s or 80s.

Outcomes

The mortality rate for an experienced surgeon is surprisingly low. Dr. Sardi comments, ?In about 400 procedures, we had only one mortality and that was in 1998. Very few treatments have as good outcomes as HIPEC, yet it remains underutilized.?? A study in the Journal of Clinical Oncology found that merely 4% of patients with colorectal cancer and peritoneal dissemination who were given only chemotherapy were alive after five years.? A prospective randomized trial in patients with colon cancer using HIPEC in addition to chemotherapy showed a 45% survival at 5 years if the cancer was completely removed.

?For patients with Stage IV cancers that have spread to the peritoneum, very few treatments have outcomes as good as HIPEC. In fact, a patient I treated in 1994 is still alive today,? concludes Dr. Sardi.

Mohs Surgery: A 99% Cure Rate

Annapolis dermatological surgeon Lisa Renfro, M.D., has performed more than 15,000 surgical excisions and 10,000 Mohs surgical procedures in her nearly 20 years in practice. Even for a dermatologic surgeon, it?s an astounding indication of the prevalence of skin cancers. With 3.5 million skin cancers diagnosed annually, it is the most common cancer in the U.S., affecting more people each year than breast, prostate, lung and colon cancers.

Mohs surgery uses microscopic examination of skin cells to trace the skin cancer and completely remove it. Performed under local anesthesia, the tumor is excised with a narrow margin and immediately processed in an on-site histological lab. There, it is color coded with dyes and the tissue is precisely mapped. Under the microscope, the tissue is analyzed for remaining cancerous cells; when indicated, a second thin layer of tissue is removed from the exact site that contains cancerous tissue. Most patients only need one stage; rarely a second or third stage is necessary to achieve cancer-free margins.

The removal of each tissue layer takes about 15 minutes, but each tissue analysis requires one to two hours due to histologic processing.? In most cases, the dermatologic surgeon performs reconstruction of the surgical defect.

The Mohs procedure offers the highest success rate of all skin cancer treatments ? greater than 99% for new cancers and 95% for recurrent carcinoma ?and preserves the maximal amount of normal tissue. Dr. Renfro remarks, ?Mohs surgery, by conserving tissue, allows the best cosmetic and functional outcome.? Since many skin cancers are on the face, that?s an important advantage.?

Mohs Surgery Indications

Basal cell carcinoma, squamous cell carcinoma and other skin cancers that:

  • Develop on areas where preserving cosmetic appearance and function are important
  • Have recurred after previous treatment or are likely to recur
  • Are located in scar tissue
  • Are large
  • Have edges that are ill-defined
  • Are aggressive and rapid-growing

Advice for PCPs

?When evaluating skin lesions, primary care physicians should be vigilant for new or changing lesions,,? Dr. Renfro advises. ?Be on the lookout for any skin lesion that is pink, scaly, or pearly, and is enlarging or bleeding. Melanomas, although usually pigmented, may be subtle in appearance and occasionally do not reveal any pigment.? Any unusual lesion should be biopsied.?

Dr. Renfro also urges primary care physicians to check patients? Vitamin D levels and to encourage patients to protect themselves from even small doses of sun exposure. ?The American Academy of Dermatology?s position is that people should get any additional Vitamin D from diet and supplements, not sun exposure,? she says.

Mohs Advances and Future Applications

Dr. Renfro comments, ?While it?s controversial, some surgeons are using Mohs for melanoma in situ, employing special immunostains. MART-1 is the stain of choice for this type of skin cancer.? Immunostaining is used in addition to H & E staining. Cytokeratin stains are used for large, advanced, recurrent cancers.?

In the future, Mohs is expected to have applications for locally invasive tumors with contiguous growth patterns in the prostate, cervix and larynx.

Cancer Survivorship Programs

With more people surviving cancer than ever before ? estimated at over 80% ? health professionals are recognizing that cancer survivors need better coordination of their ongoing post-treatment care than has been available to date. That has led to the onset of Cancer Survivorship Programs, now a required component of accreditation from the Commission on Cancer of the American College of Surgeons.

Barry Meisenberg, M.D., director of the Cancer Institute at Anne Arundel Medical Center, observes, ?Cancer patients have medical, social and psychological issues that aren?t being well addressed.? There are gaps in communication between the oncologist, primary care physician, other providers and the patient.? A plan for follow-up care is not always clear. Physicians worry they don?t have enough information on the patient?s treatment. Patients may be caught in between and don?t know where to go for screening for other medical problems. For example, who does the follow-up for back pain and how do you treat it in a patient with cancer??

AAMC?s initial survivorship program was created for those with breast cancer, but it is expanding to patients with prostate, head and neck, lung and other cancers.

Survivorship Care Plan Aids Coordination

?The survivorship program at Anne Arundel Hospital develops a detailed Survivorship Care Plan with the patient and sends it to the primary care doctors,? says Ravin Garg, M.D., medical oncologist with Annapolis Oncology Center. ?It describes the various treatments the patient has undergone and the possible long-term side effects. For example, with a breast cancer patient, it details when a patient should receive a mammogram and other follow-up tests.? All the details of the patient?s therapies, including dates and types of therapies, lymph node involvement, where the radiation was directed, what medications the patient is taking, etc., are contained in a single document.?

Dr. Garg remarks, ?It?s incredible what cancer patients go through ? typically surgery, radiation, sometimes chemotherapy and anti-estrogen therapy. Each has short and long term side effects, such as lymphedema after breast cancer excision. To patients and their loved ones, it can feel overwhelming.

?Primary care physicians should be aware of the risks of blood draws and blood pressure readings on the arm with lymphedema, but also be informed that sometimes certain exercises are no longer contraindicated for these patients,? he continues. ?Physicians also need to be aware of what a low white or red blood cell count could mean in a cancer patient (myelodysplasia from chemotherapy exposure).?

Psychosocial aspects are also part of the survivorship assessment and follow-up. Dr. Garg notes, ?Depression and anxiety are under-diagnosed in survivors. It can be difficult to address the psychosocial issues in a 20-minute visit with the oncologists, so it is important that this aspect of a patient?s longitudinal care is not forgotten.

?Nutrition is another critical and often overlooked aspect of recovery,? he states. ?I tell people to eat a healthy diet, drink lots of water and try get 150 minutes of moderate exercise every week rather than overdoing it with supplements.?

If Content Is King, Multiscreen Is The Queen, Says New Google Study

Screen shot 2012-08-29 at 18.46.43New research out from Google, working with market analysts Ipsos and Sterling Brands, puts some hard numbers behind the often-noticed trend of how people in the U.S. are using a combination of phones, tablets, computer and TVs to consume digital content. While each of these has a significant place in our consumption today, their real power lies in how they are used together -- in combination, 90% of all of our media consumption, or 4.4 hours per day, is happening across all four (which doesn't leave much room for paper-based books and publications; or for radio). This not only has implications for how content is designed, but also for how companies like Google will continue to hedge their bets across all four screens.

my big, fat road trip wedding

I held out as long as I could, but it was bound to happen sooner or later...

I?ve lived with Katie for the past three years in a great house on the west side, near an endless amount of trails and fine drinking establishments.? She has always signed off on my goofy adventures, often crewing and pacing during races or driving and hiking a different route to set water and food out for some of my longer slogs.? My crew chief, as we would say in the ultra world.

We finally decided to make it legal.? Game over, right?? But in one last desperate attempt for a good time, I scammed Katie into having the wedding in Vegas.? With lots of driving/hiking/camping before and after.? Instead of going through the hassle of a typical wedding, I told her a road trip theme would be a lot more fun.? We?ve always had a draw to the Colorado Plateau, and we came up with a plan to incorporate that into a ten day trip.? The schedule:

?

Sept 20th (Thurs) ? Leave after work, hit Moonlight Pizza in Salida, car camp at the UFO Watchtower in Hooper, CO.

Sept 21st (Fri) ? Drive to Flagstaff, AZ.? Lunch at the Durango Diner and El Rancho Saloon.

Sept 22nd (Sat) ? Hike Humphries Peak, the state high point of Arizona.? Then drive to the Grand Canyon.

Sept 23, 24, 25 ? Chill out at Havasaupai Falls, including the 14 mile hike out and back to the Colorado River.

Sept 26th (Wed) ? Drive to Vegas. ?Report immediately to the happiest place on earth, the Double Down Saloon.

Sept 27th (Thurs) ? Wedding day, tying the knot beneath the Fabulous Las Vegas sign.? She even agreed to a late ceremony so I could check out Red Rock Canyon that morning.

Sept 28th (Fri) ? Hike the Narrows in Zion Canyon.

Sept 29th (Sat) ? Morning hike of the Below the Rim trail in Bryce Canyon. ?Afternoon hike in Canyonlands.

Sept 30th (Sun) ? Hang out in Moab.

Oct 1st ? Head back to a place cooler than anything listed above, Colorado.? Hopefully the dog remembers who we are.

To my surprise, she not only agreed to this, but seems pretty excited about the whole thing.? Which is amazing, since most of the trip will have us either sleeping on the ground or in the car.? Not exactly the wedding most women dream about.

?

We will have friends and family joining us for parts of the trip.? Some hanging with us for the whole thing, some just taking advantage of an excuse to visit Vegas.? All of them thanking us for not making them dress up or blow a wad of cash at some bed, bath, & beyond register type place [side note ? I tried to register at Cheer?s Liquor, but the clerk gave me a weird look and said they didn?t do that].

Not your typical post for this site, but I'm sure the places mentioned above will make most of you jealous. ?The post-Pikes depression is over and NYC training has started, though most training plans don't include a ten day break with a little over a month before race day. ?Of course, if I stink up the Big Apple anything like I did on our mountain, I'm going to blame it on marriage.

Oldies but Goodies- Two of my Favorite Vegan Recipes ? Bliss ...

Good Morning and Happy Monday!

I hope everyone had a great weekend. ?We survived the Vancouver Soccer Slam, as well as, the Cathlamet Downhill Corral. ?We divided and conquered. ?Steve took Emmet to the 3-Day Longboarding event in Cathlamet and Zoe and I learned our way around Vancouver/Camas, WA.

Now the countdown begins, we have one more full week of summer before school starts. ?The kids are ready for school and I am too! ? I am working on a few projects that need a bit more attention and feel like I may get that once the kids are safely ensconced in school again : ) ? One of the projects I am working on is my next webinar series: ?The Art of Eating: ?Eat Your Way to Health and Happiness. ?This webinar is all about adding joy and pleasure to your eating experience and will include my 20 Pantry Essentials, 5 Simple Tips to Get Healthy and Slender, The Magic of Greens, as well as a bunch of healthy recipes!

Speaking of webinars, my Introduction to a More Earth Friendly Diet is going well and we are having a lot of fun experimenting with plant-based meals. ?A few of our favorite recipes so far are:

The ?Best? Beet Burgers and The Butternut and Tri-Bean Burrito.

The ?Best? Beet Burgers


Ingredients
1 1/2 cup cooked quinoa?
2 cup raw beets, peeled and grated
1? onion (finely diced)
4 cloves of garlic (grated or finely diced)
1 cup sunflowers seeds
1/2 cup sesame seeds
1/4 cup canola oil
1/4 cup rolled oats
2-3 tbsp whole wheat pastry flour
1/4 cup chopped parsley
2 tablespoons of Bragg?s Liquid Amino Acids
2-3 dashes hot sauce
Dash of smoked sea salt
1/4 tsp white pepper

Directions?
Preheat oven to 350 degrees. In a large bowl combine, beets, cooked quinoa, sunflower seeds, sesame seeds, oatmeal, flour,oil, parsley, and seasonings. Form the mixture into patties and bake at 350-degrees for about 25 minutes, carefully flipping the burgers halfway through.? Serve the burgers on a whole wheat bun or sandwich thin with your favorite toppings.? I suggest a heart healthy serving of avocado!? You can freeze the rest of the burgers and heat them up as needed.?

The Best Butternut Squash and Tri-Bean Burritos

  • 1 medium butternut squash, peeled, cubed, & roasted
  • 1.5 cups cooked brown rice
  • 1 cup chopped sweet onion
  • 2 garlic cloves, minced
  • 1 red pepper, chopped
  • 1 tsp kosher salt
  • 2 tsp ground cumin
  • 2 dashes of hot sauce
  • One 15-oz can tri-beans, drained and rinsed (1/2 cup red kidney beans, 1/2 cup black beans, 1/2 cup navy beans)
  • 3/4 cup Daiya Vegan Shredded Cheddar Cheese
  • 4? whole wheat tortillas
  • Garnish with fresh avocado, sunflower seeds and salsa

1. Preheat oven to 425F and line a cookie sheet with parchment paper. Drizzle olive oil on squash and sprinkle salt and pepper. Toss to coat squash. Roast chopped butternut squash for 45 mins. or until tender.

2. Cook brown rice

3. In a large skillet over medium-low heat, add oil, onion, and minced garlic. Saute for about 5 minutes, stirring frequently. Now add in salt and seasonings and stir well.

4.? Add chopped red pepper, tri-color beans, and cooked rice and saut? for another 10 mins. on low.

5. When squash is tender remove from oven and cool slightly. Add cooked butternut squash to the skillet and stir well. Add cheese and heat another couple minutes.

6. Add bean?filling to tortilla along with toppings.? I highly recommend adding the sunflower seeds for a little extra crunch. Wrap and serve.?


Oh my, these are both so good and real crowd pleasers. Yay!


In between soccer games, Zoe and I have been busy de-cluttering the house. ?One of our main accomplishments was cleaning out the refrigerator. ?We literally took everything out, disinfected the fridge and organized the contents. ?I can?t tell you what a difference this made. ?We are able to see what we need to eat down on, what we need to replenish and realize that we have a lot of condiments! ?We have a whole shelf of asian condiments that range from miso pastes to curries to soy sauces. ?


Today?s project is to de-clutter the pantry- wish me luck!


Since all of my family is going back to school next week (including my husband who is in law school) we have decided that each member of the family gets to pick their favorite restaurant to go out to eat this week. ?We have a busy Fall schedule and we are trying to eek out a little more fun before we fall into our school routine. ?

Hertz buying Dollar Thrifty for about $2.3 billion

(AP) ? Hertz is one-step closer to its long-awaited prize.

More than two years after its original bid, it agreed Sunday to buy Dollar Thrifty Automotive Group Inc. for about $2.3 billion, giving it more ways to attract travelers and expand its international presence.

At $87.50 per share, the deal is worth far more than any of Hertz's previous bids and about 8 percent higher than Dollar Thrifty's closing price Friday.

Nothing will change immediately for consumers. Travelers that rent through Dollar Thrifty will still visit that counter for service. In the long run, prices in many markets will almost certainly rise as the two companies streamline their operations.

Media reports had said last week that Hertz Global Holdings Inc. of Park Ridge, N.J., was considering a new bid for Dollar Thrifty, a Tulsa, Oklahoma-based company it had pursued for two years.

The push-and-pull between two of the nation's largest car rental companies started in 2010. Avis Budget Group was also in the mix, pursuing a bid for Dollar Thrifty for more than a year.

Avis dropped its bid nearly a year ago citing market conditions. Then, in October of last year, Hertz dropped its bid, too. But Dollar Thrifty didn't trust that the years of attempts were over. In February, it extended its shareholder rights plan known as a "poison pill" ? a maneuver designed to deter any unsolicited attempts to take over the company ? through May 2013.

Hertz Chairman and CEO Mark P. Frissora said in a statement that the Dollar Thrifty buyout will give it access to two well-known rental car brands ? Dollar and Thrifty ? as well as make it a more competitive player in Europe and other markets overseas.

"We are pleased to have finally reached an agreement with Dollar Thrifty after a lengthy - but worthwhile - pursuit," he said.

Dollar Thrifty President, CEO and Chairman Scott Thompson said that the transaction is not only compelling for its stockholders but also will help broaden its reach.

"Hertz is the logical partner for us with the resources to expand our value focused leisure brands in key car rental markets around the world," he said.

Hertz anticipates that the acquisition will result in at least $160 million in annual cost savings, with additional sales growth opportunities.

Hertz is also selling its Advantage business to rental car company Franchise Services of North America and Macquarie Capital for undisclosed terms. The Advantage deal depends upon Hertz completing the Dollar Thrifty buyout.

Both Hertz and Dollar Thrifty's boards have unanimously approved the deal, which still needs antitrust clearance from the Federal Trade Commission. Hertz said it has stayed in close contact with the FTC to secure clearance and that Dollar Thrifty will fully cooperate with the process.

Hertz has about 8,650 corporate and licensee locations in approximately 150 countries in North America, Europe, Latin America, Asia, Australia, Africa, the Middle East and New Zealand. Dollar and Thrifty have approximately 280 corporate locations in the United States and Canada. The company also has about 1,300 franchise locations in 82 countries.

Hertz shares gained $1.46, or 11.1 percent, to $14.61 in midday trading Monday. Dollar Thrifty shares rose $5.99, or 7.4 percent, to $86.99.

Isaac hugs Cuba coast; Cat 2 hurricane likely in Gulf

Florida's governor declares a state of emergency as residents and tourists flee Key West. Storm preparations are under way all along the Gulf Coast. NBC's Thanh Truong reports.

Updated at 6 p.m. ET:?Tropical Storm Isaac was hugging the northern coastline of eastern Cuba on Saturday after claiming at least four lives in Haiti. Isaac should become a Category 1 hurricane on Sunday just as it nears the Florida Keys, the U.S. National Hurricane Center said, and then grow into an even stronger Category 2 storm with 100 mph winds.

Isaac "could be significantly stronger than currently forecast" once it enters the Gulf of Mexico,?the center said in an advisory.

It will first sweep past southwest Florida and the Florida Keys, where "hurricane conditions are expected ... Sunday," it said in a separate update.


Republicans effectively cancel first day of convention

Isaac is a massive storm, with tropical storm-force winds extending 230 miles from the center. Key West International Airport was halting all flights at 7 p.m. Saturday until the storm had passed.

Tropical Storm Isaac is picking up steam as it barrels through the Caribbean. The Weather Channel's Mike Seidel reports on the storm's effects.

In Haiti, a woman and a child in the town of Souvenance were killed in the storm, a local official reported. A?woman in the southern coastal city of Jacmel was crushed to death when a tree fell on her house, government officials said.

In the capital Port-au-Prince --?where some 350,000 people are still living in tents or shelters after the 2010 Haiti earthquake --?a girl, 10, was killed when a wall fell on her.

Power outages and flooding were reported as Isaac moved across the hilly and severely deforested Caribbean country.

"There's a lot of rain, a lot of wind," said Magdala Jean-Baptiste, who huddled with her frightened children in their home in the southern coastal city of Jacmel. "We haven't had any power since the storm started yesterday. We passed the night with no sleep."?

Tropical Storm Isaac lashes the island of Hispaniola, killing at least three people in Haiti, where thousands still live in tents after an earthquake over two years ago. NBC's Mark Potter reports.

In neighboring Dominican Republic, Isaac felled power and phone lines and left at least a dozen towns cut off by flood waters. The most severe damage was reported along the south coast, including the capital Santo Domingo, where more than half the city was without power.

Cuba prepared by closing beaches and evacuating tourists in vulnerable areas, NBC's Mary Murray and The Weather Channel's Mike Seidel reported from Havana. Flights across Cuba were also suspended.?

In Baracoa, a city on Cuba's eastern side, high seas began topping the seawall Friday night, Radio Baracoa reported.?

Now with 60-mph winds, Isaac should exit Cuba on Sunday and then move south of the Florida Keys and into the Gulf.

Libya ministers under fire over security

National assembly members have called for the sacking of Libya's defence and interior ministers over the security situation in the country, a member told AFP.

"There have been demands for the dismissal of the defence and interior ministers but the assembly has called for them to be summoned first to hear them. This should happen later today or tomorrow," he said, asking not to be named.

He said Prime Minister Abdel Rahim al-Kib was also being summoned.

On Thursday, the national assembly, which was elected in July, met in closed session to discuss security problems across the country, with Interior Minister Fawzi Abdelali reportedly coming in for heavy criticism.

The session followed a double car bombing in Tripoli on August 19 that killed two people. Tribal clashes in the western region of Zliten have since left at least two dead and several wounded.

The assembly's speaker, Mohamed al-Megarief, who was to give a speech later on Saturday, said he tried in vain to contact Abdelali but that the interior minister's mobile phone was "switched off."

Rumours have been circulating in Tripoli for days that Abdelali and Defence Minister Osama Juili could both face the sack.

How do you know when it's been enough? - Talk About Marriage

My question is a very common one, probably a clich?d one. But I've got to do something or I'm going to go crazy! I?m going to begin this the way I see a lot of women begin these types of letters. My fiance is great, amazing, respectful, and a really good match for me. But.

But I?ve always thought of relationships as having two parts: one part is the love, and the other is the reality. The love part between us, the personal relationship, the sex, trust, emotional stability and security, the things we have in common, is great. The reality part is not so much.

What I mean by that is we have been together for over a year now. I place a very high value on practical stability and security in my life, and it is one of my top priorities not just in my personal life but in any relationship that I cultivate. Things like financial responsibility and protecting my future (in terms of practical things) are essential to my own mental well-being. This relationship that I?m in only fulfills half of what I need to be happy in a long-term relationship. He has not held down a job for more than a few months for the entire time that I?ve known him. He isn?t terrible with money, but he tends to be a lot of talk and no action. He?s going to stop smoking cigarettes to cut down on expenses ? tomorrow. He?s going to do this or that to make things better ? tomorrow. There is no sense of security or stability in the practical sense with him. I am not willing to join our households, finances, and futures unless I am (reasonably) sure that they will be safe and protected, and the fact of the matter is that I am not at all sure that this is the case with him. He tends to displace responsibility for the state of matters ? it?s always someone else?s fault that things are the way they are. I can?t be happy supporting him; maybe this is a result of outdated gender roles, but it would not take many months of me paying the bills while he hung around at home to get resentful and angry. I hesitate to say he?s lazy or uncommitted, but if I was out of work and promising someone marriage and a home, I would be busting my ass to find anything and everything. I am ambitious and driven, and I just can't imagine spending all day at a demanding job to come home to someone who hasn't done anything.

This places our relationship at a stagnant point. I?m not willing to move forward (move in together, get married, etc) until that other half of what I need is fulfilled. But I don?t know how long I am willing to wait, either. I?m at an age where I want to be moving forward in a long-term relationship toward marriage. I hate being in purgatory, and I?m tired of hearing promises that it will all work out? someday. I?m tired of putting energy and time and love into this relationship and feeling like I?m running in place. Or worse, moving even farther away from those long-term goals.

Well, you say. Easy enough. The relationship is over, he won?t change, and it?s time to break up. It seems simple. But of course, if it were, I wouldn?t be this torn. The other day, I found myself saying: ?If it weren?t for money, we?d have the perfect relationship!? So, I ask myself ? am I seriously contemplating throwing away what would be the perfect relationship because of money? Am I that materialistic? And isn?t getting half of what you need better than getting nothing at all? And I can make a list a mile long of his good qualities, and the genuine effort he puts in to making me happy. He has changed for me. He has made radical differences to his life in order to make me happy and help give me what he knows I want. And it feels ? well, churlish and ungrateful and greedy to say that it?s not enough. That I expect more, that he ought to try harder and work harder. But.

But, well ? I do expect more. I do feel that he ought to try harder. Work harder. (I want to add in here that I hold myself to the same exacting standards.) And this completes the cycle. I fluctuate between hopefulness and grim resignation. Between love and anxiety. One day everything will be okay and the next, we?re worse off than before. This emotional back-and-forth is killing me. If it were all bad, I wouldn?t hesitate to end it. But, well ? I love him. He?s a really genuinely good guy. And yet? where is our future? When will it come?

Any advice, thoughts, comments, stories, or anything at all would be very welcome and appreciated.

Jury Reaches Verdict in the Apple vs. Samsung Trial

Court official have just announced that the jury has reached a verdict in the Apple vs. Samsung trial, reports the Verge.

After a surprisingly short time, court officials have informed us that the jury in the Apple v. Samsung case has reached a verdict. We're heading into the courtroom now, and will let you know what it's decided live, as it happens.

Update:
The verdict is being read. So far the jury has found that Samsung has infringed on Apple patents with many of its devices; however, there are numerous infringement claims to go through.

It also found that Samsung's infringement was willful for numerous devices.

Another big win for Apple. The jury ruled that all its patents were valid.

Pittsburgh-area cancer survivors have reason to strut during fashion show

Walking down the runway will be about more than showing off beautiful fashion.

It will be about courage and survival, and a chance to forget about medications and doctors appointments.

The models walking the catwalk are cancer survivors and/or their family members. They are taking part in the first Strut Your Strength Fashion Show: Where There?s a Will, There?s a Runaway on Saturday night.

The event is a team effort between Gilda?s Club Western Pennsylvania and locally based online boutique ModCloth (www.modcloth.com). Gilda?s Club, which is in the Strip District, offers support programs for anyone living with cancer ? men, women, teens, children ? and their families. Its members will be modeling fashions by ModCloth, which features retro, indie and vintage-inspired clothing styles. The show benefits Gilda?s Club.

?We invited the models to try on something that they might not have worn before,? says Aire Plichta, fashion press specialist for ModCloth and a co-chair of the event. ?Maybe they could try something with sparkles, or in a color that is different than what they normally would wear. We wanted them to have fun doing this, and for it to be an enjoyable experience.?

There will be 19 models. Each chose a song that will be played as they walk the runway. Words from the models about how cancer has affected their lives will be hung on the walls. The quotes are so powerful, says Allison Franks, senior product launch coordinator at ModCloth.

?I really like this dress I am wearing,? says Olga Dubrovsky, a two-time cancer survivor from Plum, who will be wearing a multicolor dress. ?It is not really my style. I thought I should have worn black, but once I put this one on, I really liked it. It is so colorful. It is a little out of my comfort zone, but now I can?t wait to walk down the runway in it. I am so excited.?

Joining Dubrovsky on the runway will be Marla Roebuck of Crawford Square, Uptown. She has participated in many of the activities at Gilda?s Club as a way to support her sister, Margaret Antoinette Bray, who has breast cancer.

?As soon as I heard about this fashion show I thought it was a fantastic idea,? says Roebuck, who has lost three siblings to cancer. ?I have been trying to get my sister to come to events at Gilda?s. Gilda?s is a wonderful place and has helped me so much. I have done everything there from yoga to jewelry making. It is such a wonderful atmosphere when you walk through that door.?

The partnership was formed because Jen McCrady, who is on the board at Gilda?s and is a cancer survivor, knows ModCloth?s landlord. So, she and her colleagues on the board reached out to ModCloth, which has been in the Strip District before moving to Crafton. She and Franks, are chairing the event.

?This is a great opportunity for us,? says Carol Lennon, executive director at Gilda?s Club. ?Everyone from ModCloth has been wonderful. We didn?t know what to expect, but they have been amazing and so accommodating. And, to be working with a locally founded company, it was just a natural connection. It?s a wonderful partnership that we wish to continue in the future.?
